  1. You are greeted by a friendly interviewer who will document the situation with you in detail. You don’t have to give your name and your call is not recorded.
  2. The interviewer assigns you a reference number and asks you to make one scheduled follow-up call.
  3. The information is relayed to the Office of Institutional Compliance and Risk Services at UTSA where it is evaluated to determine the most appropriate means of handling the issue.
  4. Please note, the interviewer will provide you with a reference number that can be used to follow-up on your concerns. When you call back, you may be asked for additional information.
  1. You will be guided through the web reporting system step by step.
  2. The web reporting system will ask a series of questions and you will fill in the answers.
  3. You will be prompted to select which incident best describes the issue you are reporting (ie., Discrimination, Fraud, Policy Issues and others).
  4. You have the option to remain anonymous or provide your name and contact information.
  5. The information is relayed to the Office of Institutional Compliance and Risk Services at UTSA where it is evaluated to determine the most appropriate means of handling the issue
  6. Please note, you will receive a reference number when you submit your report online that can be used to follow-up on your concerns. When you check back, you may be asked for additional information which you can then submit.
