***Attn: Veterans applying for Post 9/11 GI Bill***

Only those Veterans wanting to switch to the new Chapter 33 (Post 9/11 GI Bill) need to re-apply to the VA. We are unable to certify Fall 2009 enrollment certifications for those electing Chapter 33 until we receive a new Certificate of Eligibility (COE) and a copy of your DD-214 (member 4 copy). Make note that this election is irrevocable. The new Chapter 33 (Post 9/11 GI Bill) application will be available after May 1st thru the VA website. Fall 2009 enrollment certifications can be processed after July 6th for those electing the new Chapter 33. We will be processing summer enrollments as usual. Veterans that will be continuing with their current chapters do not need to re-apply to VA and may submit their certification request forms for summer and fall after they are registered for classes.

*The forms on this webpage are interactive PDFs which require version 7.0.5 or higher of Adobe Reader in order to be properly viewed and completed. Contact us with questions.

Each semester a student wishes enrollment certification they must sign a Certification Request form. This is authorization for us to prepare enrollment certification and transmit it to the VA. We will not certify any student without this form. Students must be registered for classes before turning in this form.

If you are attending UTSA for the first time you may fill out the Acknowlegdement of Understanding (Waiver) form. This allows you one semester to bring us an evaluated and signed degree plan from your academic advisor. Chapter 31 Vocational Rehabilitation and Graduate students are not allowed to use the waiver. A signed, evaluated degree plan must be brought to us before we can certify your enrollment.

Chapter 31 Vocational Rehabilitation students must sign and submit a Checklist to the Veteran Certification Office each semester they request certification. We will not certify your enrollment without this form.

The forms listed above are no longer being distributed by this office. You may now submit these forms online. If you wish to print out the forms, you may drop them off in the office, fax, or send via U.S. mail.

REMEMBER: faxes are not always received legibly so we highly recommend that you stop by or phone our office within 24 to 48 hours to verify completion of the certification process was accomplished.
