Guards or Reservists All students using benefits must complete the UTSA
Veterans Certification Request form every semester. Chapters 30, 32, 34, 1606: VA form 22-1995 Chapter 1607: VA form 22-1995, DD 214 member #4, copy of deployment orders Chapter 35: VA form 22-5495 If you are using your VA educational benefits for the first time, you will need the following forms: Chapter 30: Member-4 copy of DD form 214, VA form 22-1990. Chapter 34 students will also require VA form 21-686C, if claiming dependents. The VA also requires copies of marriage and/or divorce records, birth certificates etc. to support dependency claims. Chapter 31: See case manager at the Frank Tejeda Outpatient Clinic, 5788 Eckhert Road, San Antonio, Texas (phone 210-699-2323), to receive 28-1905. Chapter 32: A certified copy of DD form 214, VA form 22-1990. Chapter 35: VA form 22-5490 (must know file number). Chapter 1606: DD form 2384 bearing commanders original signature and eligibility date, VA form 22-1990. Chapter 1607: VA form 22-1990, DD 214 member #4, copy of deployment orders Active Duty: VA form 22-1990 or VA form 22-1995,
depending upon previous use of benefits, including signature of Base
Education Officer. Forms can be obtained from the Veterans Certification office at either campus or using links on the VA Web Site.
