Disability Services requires each student requesting services to provide current and comprehensive documentation of disability before eligibility for services can be determined. Disability documentation for the purpose of providing accommodations must both establish disability and provide adequate information on the functional impact of the disability so that effective accommodations can be identified.

Students with hearing impairments must submit an audiological evaluation by a licensed audiologist. The evaluation report should include the following:

• The nature and severity of the hearing loss
• Functional limitations caused by the hearing loss
• The student’s ability to benefit from amplification
• Recommendations for academic accommodations
