Independent Verification Worksheet

The Federal government requires that educational institutions verify the accuracy of the information submitted on the Free Application for Federal Student Aid (FAFSA).  There are two forms, one for students who are dependent and one for students who are independent If you have answered “yes” to any of the following questions on your FAFSA application, you will be required to fill out the Independent Verification Worksheet: 

  1. Born Before 01-JAN-1986
  2. Graduate or Professional school
  3. Married
  4. Have Children you Support
  5. Legal Dependents
  6. Legal Guardianship
  7. Orphan or Ward of the Court, Foster Care
  8. Active Duty in the U.S. Armed Forces
  9. Veteran of U.S. Armed Forces
  10. Emancipated Minor
  11. Unaccompanied youth by School
  12. Unaccompanied Youth by HUD
  13. At Risk of Homelessness

If you answered "no" to all of the questions above you will be required to fill out the Dependent Verification Worksheet.
