This form is to be used to cancel all financial aid awards (including grants, loans, scholarships and stipends) in the event that you decide not to attend UTSA for the indicated term. You cannot have financial aid awarded at more than one school. The Request to Cancel All Financial Aid form is not to be used to withdraw from classes.
