New Deposit Transmittal Form
Two signatures are now required on the deposit transmittal form used by departments who make deposits to any of the Fiscal Services offices, the preparer of the deposit and their supervisor. In a recent Cash Operations Audit, supervisory review of all deposits was recommended so the supervisors might be able to identify unusual patterns and/or detect potential fraudulent activity in their departmental deposits.
All departments should begin to use the new form immediately. Fiscal Services will continue to accept the old form for deposits until October 1, 2005. After that date, departments will be required to have the supervisor signature on all deposits they submit to Fiscal Services.
