Recommended Sequence of EventsRequired Doctoral Forms: Please do not change the format of the forms below without permission from The Graduate School. These forms are to be used by the student, advisor and department for the completion of the Doctoral Degree. Selection of an advisor- This is done very early in the candidate's doctoral career within the students respective department. Interim Program of Study- This form is required and is to be established very early in the student's academic career as a Ph.D. student. The advisor should be able to fill out this form and then route it to the Graduate School for the Dean's approval. Final Program of Study- This form needs to be filled out by the student's advisor or department administrator and routed to the Dean of the Graduate School for final approval. It is filled out during the final semester of the student's doctoral career. Completion of the Qualifying Exam -This form is required to be filled out after the student has successfully completed all parts of the qualifying exam. The form must ultimately be routed to the Graduate School for the Dean's approval. Application for Candidacy- This form must be filled out by the student and advisor in order to establish that the student is ready to begin working on his/her dissertation. This form must ultimately be routed to the Graduate School for the Dean's approval. Appointment of Doctoral Dissertation Committee- This form is to establish a dissertation committee for the student. This form must ultimately be routed to the Graduate School for approval. NOTE: If the student is using an outside committee member, the Application for Graduate Faculty Special Membership must be submitted to Graduate School,allowing sufficient time for processing by the Graduate Council Membership Committee and approved by the Graduate Council. The Graduate Council meets the second Tuesday of each month from September to May. External Committee Member and Tentative Dissertation Title Memorandum- This is the only document that does not need to be routed to the Graduate School for approval. This form is filled out by either the advisor or by a administrator within the department in order to establish that an outside member has agreed to serve on a student's dissertation committee. Dissertation Proposal Approval Form- Submit Dissertation Proposal to Advisor, Dissertation Committee and, finally, to the Graduate School for the Dean's approval. Complete research, write dissertation, and get Dissertation Committee approval. Submit preliminary draft of dissertation to Graduate School for approval- This step is not required (read more here ) but it is highly recommended that students be aware of Graduate School deadlines and dissertation formatting guidelines for in order to get their dissertation approved. Defend Dissertation- Students are to meet with their Supervising Professor and dissertation committee in order to decide upon a date for defense. Certification of Completion of Dissertation Requirements for Doctoral Degree Your department will complete this form once you have met all requirements to graduate. This form must ultimately be routed to the Graduate School for the Dean's approval. Submit final copies of dissertation to the Graduate School- PhD Candidates are required to submit two hard copies of their dissertation to the Graduate School and one electronic copy to the ProQuest UMI website.
