	EMPLOYEE SEPARATION NOTIFICATION ACCESSING SEPARATION NOTIFICATION FORM
For security purposes, the web address, (URL) for access to the Separation Notification Form is located in Outlook. Here's how to find it. Log on to the UTSA Network and open Outlook. Click on Public Folders, which is located on the left side of the Outlook menu screen. Navigate to / All Public Folders / HR / Finally click on the UTSA Separation Process memo that's located in that folder. You will find the link to the Separation Notification Form in the memo. (Note: If you do not see a folder list on the left side of your Outlook window, go to the menu bar at the top of the window and select View, Folder List, and then click on Public Folders.)
