If an employee:
- Is off work due to illness or injury for 3 or more calendar days.
- Anticipates an absence of 3 or more calendar days due to their illness, injury or pregnancy,
or that of a qualified family member.
- Has an attendance problem (prior to recommending termination or disciplinary action).
- Has developed a "pattern" of absences.

Contact Human Resources at 458-4031 or 454-7874. The designated Human Resources staff member will ask you questions about your illness or reason for taking leave and send you the paperwork you will need to complete and return as soon as possible.
Notification:
Generally an employee must provide 30 days advance notice when possible to the department of the need to take leave, or as soon as possible. Employees must provide a Physicians Statement of their medical status or that of their qualified family member, within 15 days of an employers request. The Physicians's Statement must include the effective date of leave and anticipated duration of the leave.

Lost time covered under FMLA:
At UTSA, FMLA runs concurrent with sick time and vacation time, STD (Short Term Disability) and Worker's Compensation. Therefore, the employee will exhaust sick / vacation time while on FMLA.
FMLA Approval:
- For both faculty and staff, approval is made by the designated FMLA Human Resources staff member
(in coordination with the employee's supervisor).
- Appeals on denials for FMLA should be addressed with an FMLA
HR staff member.
