Please complete the Change of Status form when you have a qualifying family status change occur and need to make changes to your benefit elections (see change of status form for examples of qualifying events. If you have questions or need assistance completing the forms, please send an email to benefits@utsa.edu. Benefits Cost Worksheet link to help you estimate the cost changes. Keep in mind that you only have 31 days from the date of the change to complete these forms and provide documentation of your status change to the benefits office.
