| Group Health Insurance | ||
The federal law known as COBRA requires most employers to offer employees and their families the opportunity for a temporary continuation of their group health insurance coverage at group rates (without State Premium Sharing) when the employee terminates employment. You may apply for COBRA up to 60 days from the date you lose coverage or date you terminate employment. Please call the Human Resources Office for an appointment or group rate information, prior to your last work day, if you wish to continue your coverage. Life Insurance and Long Term Disability (LTD) at termination, if you are covered by term life and long term disability that protection will end. You may convert or continue your life insurance or long term disability insurance. Call Human Resources to request rates and the conversion or continuation request form. Application and payment must be received by the insurance company within 31 days from date of termination. |

| Teacher Retirement (TRS) | ||
If you have completed the TRS-6, Notice of Final Deposit and Request for Refund Form, allow 12 weeks (90 days) following receipt of this form in the Payroll and TRS offices. The Teacher Retirement System will mail the refund check to the address specified by you on the TRS-6 Form. Should this address change for any reason you will need to contact the Teacher Retirement System at 1-800-223-8778 to ensure delivery to the correct address. |

| Optional Retirement Plan (ORP) or Tax Sheltered Annuity (TSA) | ||
If you are currently participating in the Optional Retirement Plan (ORP) or in the Tax Sheltered Annuity (TSA) program, you should consult your representative or the company directly for any information concerning distribution of funds. |

| Unused Vacation hours, Floating holidays and Sick Leave | ||
If eligible, your unused vacation hours will be paid to you in a lump sum in approximately 30 to 45 days from your last day on payroll/termination date. Please remind your department to process the paperwork to compensate you for any unused vacation hours and floating holidays, provided you do not become employed with another State Agency or U.T. Component. Upon separation from employment for any reason, you will not be paid for any accrued sick leave. Unused sick leave hours are forfeited, unless you return to State employment within 12 months of termination, provided there has been a break in service of at least one month, since the date of termination. |

| Separation Processing Checklist | ||
You will need to complete and separation processing checklist form on your last day on the job. You should request this form from your department 2 or 3 days prior to your last day to ensure they have sufficient time to prepare prior to your last day on the job. |

| Final Paycheck | ||
You will receive your final paycheck for hours worked from the Payroll Office. If your paycheck is normally deposited directly to your bank, this authorization will continue. Distribution instructions (pick up in department, mail or continue direct deposit) can be specified by you on the separation processing checklist. |

| W-2 Statement | ||
It is very important to keep Human Resources informed of any address changes to ensure proper delivery of next year's W-2 statement to your home. To report a change in your address you can mail a statement with your social security number, your signature and the new address. You may fax your change to Human Resources at (210) 458-4647.Unemployment Compensation Insurance Weekly unemployment benefits may be payable to former employees who were separated from employment with the University of Texas at San Antonio under certain conditions specified in the Texas Unemployment Compensation Act and who are unable to find suitable employment. Application for unemployment benefits may be made by contacting any office of the Texas Workforce Commission (formerly Texas Employment Commission). |
