Network/E-Mail Account Request Form

A campus network account request form is required for all members of the UTSA Community that want access to the UTSA computer network.

Please complete the online form, and acquire the required signatures on page 2. 

You may deliver the form to SB 1.01.10, campus mail the form, or FAX the signed form to the Help Desk at 458-5741. If the form is sent via FAX, the original form must also be sent to the Help Desk to keep on file.
