Health Insurance Waiver

Are you covered by another health plan that meets the requirements of UTSA policy? 

In order to submit the insurance waiver, you must meet or exceed the requirements.  Please view the Insurance Eligibility Checklist link below to help you determine your eligibility.

Once you have determined that your eligibility, complete the Insurance Waiver Application below and bring it to the Office of International Programs with documents/insurance proof of coverage to include dates of insurance coverage by the Census Date.  Documents must be in English with U.S. currency. 

For the Summer 2008 Semester, the Office of International Programs will accept requests from 9:00 am to 4:00 pm each regular business day beginning May 1, 2008 through June 11, 2008.
