Health Insurance Waiver

 

 

Are you covered by a health insurance plan other than the mandatory UTSA insurance policy?  If yes, you may be eligible to apply for a Health Insurance Waiver.

 

In order to apply for an insurance waiver, you must meet or exceed the requirements listed on the Insurance Eligibility Checklist. To determine your eligibility, you must complete page 1 (Insurance Eligibility Checklist) of the Health Insurance Waiver Application.

 

Once you have determined that you are eligible, you may continue to complete the Health Insurance Waiver Application (page 2 and 3) and bring all 3 pages to the Office of International Programs with copies of your proof of insurance coverage documents by the Census Date.  Documents must be in English with U.S. currency. 

 

HEALTH INSURANCE WAIVER APPLICATION

 

For the Summer 2009 session, the Office of International Programs will accept applications for Insurance Waivers from 9:00 am to 4:00 pm each regular business day beginning May 1, 2009 through June 16, 2009.
