Federal form W-9
The funding from the Pat and Tom Frost Foundation provides a small stipend for
mentors who work with GE2MS students during a semester. In
order for UTSA to cut a check, every Mentor needs to submit a W-9 in order to be
entered into the financial system of UTSA. The W-9 is only submitted once,
unless there are future changes in address, name, etc.

Please sign and submit an invoice each semester you work with GE2MS
students.
