The FY 07-08 Annual Physical Inventory cycle will kick off beginning
6 Nov 2007.
The
new DE 676 Inventory Palm Scanner "Refresher" course will soon be offered
online through the Human Resources Training and Development Learning
Source. Keep on the lookout for future updates!
Below are revised FY 07-08 forms; please use these forms for future Inventory correspondence.
Inventory
Transaction form (WILL NOW INCLUDE TRANSFER OF EQUIPMENT
REQUESTS) - The Inventory
Transaction form has been revised once again for FY 07-08. You can now
use this form to Transfer Equipment, Request a Replacement
Barcode Label, report Items Not on Departmental Inventory, Request
Description Changes, and Request Location Changes. (September 2007)
Inventory
Contact Person (ICP) Appointment form - The new FY 07-08 ICP Appointment form is
now available in our
forms section. Thank you for
submitting your form within the requested time frame.

Missing Property/Reinstatement Report - In an effort to eliminate
the many forms we require; we have combined two popular forms into one.
The Missing Property Report and the Reinstatement of Missing Property
are now on the same form; renamed the Missing
Property/Reinstatement Report. Use this form when reporting an
asset missing or when a previously reported missing asset is found. This
form is now available in the forms section.

Stolen/Recovered Property Report -
We have
also combined the Stolen Property Report with the Recovered Stolen Property Report.
Use this form when reporting an asset stolen or if a previously stolen
asset is recovered. This form

Associate Vice President Financial Affairs Stolen Property Review
Committee
- When an
item is reported stolen, the Inventory Office is required to forward
copies of the UTSA Police Department/Offense/Incident Report/UTSA
Form #21 or a Local Police Report, plus the Inventory
Stolen/Recovered
Property Report
to the Associate Vice President Financial Affairs. The Stolen
Property Committee will render the final decision on negligence.
