The Conduct Referral Form for behavior is used by Students, faculty and staff to relay information regarding a possible violation of the Student Code of Conduct or a student concern that does not relate to scholastic dishonesty to Student Conduct and Community Standards. The Faculty Disposition of Scholastic Dishonesty Case form is used by Faculty to initiate disposition or resolution to any issue of Scholastic Dishonesty related to class. When scholastic dishonesty is encountered or suspected, this form should be filled out by the faculty member and used to meet with the student. It should also be signed by a faculty member’s chair of dean. The No Report Form is used to establish a non-punitive grade replacement while a Faculty Dispositions of Scholastic Dishonesty is pending. No Report Forms are used most often when the incident occurs near final grade time. Faculty are responsible for notifying their Academic Dean’s office when grades are submitted, noting that pending the outcome of a scholastic dishonesty case, an “NR” should be posted for the student’s grade. The Academic Dean’s office prepares a memo, which is then forwarded with the grades to the register’s office, who records the NR. (NR stands for “No Report”). Faculty members are asked to use the No Report Form any time final grades will be reported during the fourteen (14) day appeal window. If the student does not exercise his/her right to appeal, grades may be posted on the fifteenth (15th) day.
