Submit a Report

This site is provided for the University Community to easily and quickly report critical information to The University of Texas at San Antonio. Each one of the links below will direct you to a specific form and each form will have directions about the intended purpose, and/or limitations associated with its use. Once submitted, the appropriate university officials will be notified.If you have questions or concerns, or are unsure of how to best report your concerns, please contact Student Conduct and Community Standards at 210-458-4720.

Forms should be filled out with complete, accurate, and specific information. Please know reporting anonymously, or with incomplete or inaccurate information, may impair, deter, or delay the University's ability to take the appropriate action needed.

Report alleged violations to Title IX, Sexual Misconduct and Harassment, or other types of discrimination that have occurred involving a student.

Submit an appeal to the Senior Vice President and Provost of Academic Affairs for Behavioral and Scholastic Dishonesty cases.

Report general incidents,
complaints, or concerns.


Report concerns or actions
(experienced or observed) that you believe pose a risk of harm or
danger to self and/or others.

Report any alleged behavioral or scholastic violations of the Student Code of Conduct or University Policies.
