Academic Forms
Academic Affairs agreement forms can be accessed on UTSA's Academic Agreements webpage managed by Strategic Planning and Assessment. All forms are to be routed for signature via the CourseLeaf system (see more information below).

Submitting Forms
CourseLeaf is the system used to submit requests. Proposals will be routed based on the appropriate workflow for the proposed academic program change or academic agreement. Users have been assigned to each college and/or administrative unit.
