Academic Forms

All Academic Affairs agreement forms can be accessed on UTSA's Continuous Improvement & Accreditation webpage, and must be initiated and routed via the CourseLeaf system. Once initiated, the system will route to the Office of Legal Affairs for review and approval, as required. See Academic Agreements.

CourseLeaf is the system used to submit requests. Proposals will be routed based on the appropriate workflow for the proposed academic program change or academic agreement. Users have been assigned to each college and/or administrative unit.
