Disability Accommodations for Job Applicants and Employees

Employees requesting an accommodation under the Americans with Disabilities Act (ADA) will submit their request by completing the accommodation request form, as well as the medical inquiry form, which can be found on the ADA Website. Once completed and provided to the ADA office, the ADA Coordinator will reach out to discuss next steps.
