The CBC form can be sent to Human Resources by fax to (210) 458-8586, by campus mail (Attn: CBC), or hand delivered to the Human Resources front desk at the University Heights office.
Departments may also wish to send their applicants to Human Resources to have the CBC form filled out; however Section A will need to be filled out by the department.
REMEMBER: Incomplete or incorrect forms will be returned to the hiring department and will result in a delay in processing!
