Criminal Background Check (CBC)

All Categories Must:

 
  • Verify the information on the CBC form with a state or federal picture identification document (i.e. driver’s license, passport, state identification card)
  • Complete the verification portion by listing:
      • Type of state or federal picture ID
      • Document number
      • Verifier’s name

Once an individual is identified for a position that requires a CBC, determine which form will need to be completed based on employment categories listed below: A, B, or C.

Category A

    • Applicant (finalist) - external applicants who applied through STARS
    • All newly assigned (hired) faculty, post docs and staff  (including part time and temporary)
    • All newly assigned (hired) University affiliated workers (a worker with access to UTSA resources/paid through private, federal, or other state agency (e.g. UTHSCSA professor)
    • Occasional Worker or Volunteers (Overnight travel with staff or students, Access to UTSA resources and requires an HRMS Assignment)
    • 1099 Contractor (independent contractor hired by UTSA) 

    (Newly hired/assigned = No assignment within 1 year in HRMS)

    Category "A" form [ Link ]

    Category A Instructions:
    This link contains one 2 page document. Return the completed and signed document to UTSA Human Resources.

    • The department will fill out Section A online completely and legibly.
    • Applicant or employee will fill out Section B completely and legibly.
    • Applicant or employee must sign CBC request
    • Fax the CBC form to Human Resources at (210) 458-8586
    • The UTSA Police Department will communicate the results of the CBC to the hiring official once the CBC is complete.

    Georgia form [ Link ] (Category "A" only)

    Only used if applicant has a Georgia Home or Work address

     



Category B

    • Internal (Finalist) Applicant-Current employees who applied through STARS
    • Applicant and newly hired/assigned employees in student titles

    Category "B" form [ Link ]


    Category B Instructions:
    This link contains two separate documents.

    • UTSA CBC
    • DPS Verification

    Return both documents completed and signed to UTSA Human Resources.

    • The department will fill out Section A online completely and legibly.
    • Applicant or employee will fill out Section B completely and legibly.
    • Two signatures required by Applicant or Employee:
      • CBC request
      • DPS Verification (Only HR and Applicant should fill in this form)
    • Fax the CBC form and the DPS Verification form to Human Resources at (210) 458-8586

Category C

    • Volunteer 
        • With no HRMS assignment
        • Who do not travel overnight with staff or students
        • No Network Access
        • No Access to other UTSA Resources
    • Occasional Workers
      (Occasional worker (works sporadically,  no more than 80 consecutive hours, e.g. athletic or activities assistant)  
        • With no HRMS assignment
        • Who do not travel overnight with staff or students
        • No Network Access
        • No Access to other UTSA Resources
    • Staff or student rehire/reassignment with a title or department change or break after 4 months but less than 1 year) (e.g. Reclass/transfer/promotion);  does not include progressions in a job family

    • Faculty reassignment  (with a break in service of greater than 4 months & less than 12 months)

    NOTE: If rehired/reassigned after a 12 month break, person must have a CBC consistent with category

    Category C Instructions:
    Fill out information on the spreadheet below and email to hrcbc@utsa.edu

    • No signature is required
    • No limit to the number of individuals on one spreadsheet
