CBC Request Form
Using Microsoft Word 2003
Inital Access / Troubleshooting Drop Down Menus
If you are accessing the CBC - Criminal Background Check Form for the first time or are experiencing problems with the drop down menus, you must verify the settings on your computer are set correctly.
Follow the 2-Step Process below
NOTE: This is a QUICK ONE-TIME process and DOES NOT need to repeated again for any HR forms.
STEP 1:
Open the CBC Request Form.
Select the Open button

If you are prompted with the following window, this indicates that your settings are correct!
Select the Enable Macros button and proceed using the CBC Request form.

At this point the document will open and the drop down boxes for department name and hiring official will be functional. The CBC form that you access via the HR web site is now operational..
STEP 2: If you are NOT prompted with the "Macros" window above:
Once the document is opened, the Department drop down list under Section A.
will be incorrectly populated with a zero as follows:

Proceed with the following process:

At this point the document will open and the drop down boxes for department name and hiring official will be functional. The CBC form that you access via the HR web site is now operational..
