Instructions for New Non-Benefits Eligible Employees

Designed for use by Non-Tenure/Track Adjunct Faculty, Student,
Part-Time, Temporary & Work-Study employees

A Criminal Background Check (CBC) may be a requirement BEFORE an individual can start work. Go to the UTSA CBC web page for instructions on the CBC process.

 

There are a series of forms and reading materials required to validate the hiring process and establish the employee and payroll record for non-benefits eligible employees. The forms and reading materials apply to NEW, or former UTSA employees returning after a 1-year or more break in service. The table listed below contains specific instruction regarding the completion of these forms and in addition, provides information relating to former UTSA employees returning with less than a 1-year break in service.

Instructions
Selective Service Form For males only. Employee completes per the instructions. If employee is required to be registered with the selective service, a copy of the employee's selective service registration is required.
I-9 Employment Eligibility Verification Employee completes section 1.
Complete Section 2, based on documentation the employee has provided.
Forward competed form to Human Resources.
This form must be completed and certified within 3 business days of hire.
W-4

Employee completes per the instructions on the form.

Former UTSA employees returning after a break of 120 days or more are required to complete a new form. Former employees returning after a break of less than 120 days need not complete a new form but should review the contents of their existing form to insure it contains the correct withholding information, addresses etc.

Direct Deposit Employee completes the form only if he/she desires to have pay directly deposited to a bank or credit union.
Former UTSA employees returning after a break of 120 days or more are required, if they elect direct deposit, to complete a new form. Former employees returning after a break of less than 120 days need only to complete a new form if they want to make a new election or change the information on their previous form.
