A campus network account request form is required for all members of the UTSA Community that want access to the UTSA computer network.

Please complete the online form and acquire the required signatures on page 2.

You may deliver the signed form by campus mail, via FAX at 458-5741 or in person to PP 2.112.

If the form is sent via FAX, the original form must also be sent to OIT Support Services (Help Desk) (Physical Plant Building, 2nd floor) to keep on file.
