Order or Upgrade Telephones
A Telephone Services Work Request (DOC) is required to order or upgrade a phone.
The form must include the appropriate account number of the department to be charged for the services requested.
The administrative head of the office, college or division (or the designated person of authority) must sign the Telephone Services Work Request (DOC) form. Faxed copies are not accepted.
The Office of Sponsored Programs must approve all Telephone Services Work Request forms using grant "26" funds.
