Annual Performance Evaluation

  • For this transition year, we will extend the current performance review period through August 31, 2020. Therefore, this year’s performance evaluations will cover the period February 1, 2019 – August 31, 2020, and the forms will be due to Human Resources on October 1, 2020. We strongly encourage supervisors to continue meeting with employees regarding their performance over this extended period.
  • Evaluations are due to HR by October 1, 2020.
  • The supervisor must place the original evaluation form in the employee's departmental personnel file and provide a copy to the employee and a copy to the Office of Human Resources no later than October 1, 2020.

Rating Tool

Delivery Method:

1) Supervisor will fill out Staff Evaluation Form and discuss with employee

2) Once the final evaluation is complete prepare the document for signatures

a) Under the “Home” tab in the ribbon above click on the “Request Signatures” Icon on the far-right corner of the ribbon; creating a PDF file
b) Save the PDF file using the employee’s 10-digit Employee ID as the file name
c) Once the PDF document opens sign the document

3) To sign the document:

a) Scroll down to the signature area of the PDF file and click on the “Fill and Sign” option to open up the signature option; this can be done using either the “Tools” option on the ribbon or the fountain pen icon of your Adobe Acrobat Pro DC
b) Save the document and send to the next person to be signed
c) Repeat steps 3a and 3b
d) Once all parties have signed the document return the document to the primary supervisor

4) Place the original evaluation form in the employee’s departmental personnel file

5) Provide a copy via email to the employee

6) Send a signed copy of the PDF file to your  HRBP

Probationary Employee Evaluation

A newly appointed regular classified employee who is on probation as described in UTSA Handbook of Operating Procedures: 3.06 Probationary Period For Classified Employees will require periodic performance evaluations at 30, 90 and 160 day intervals.  The appropriate forms to be used for this purpose are listed below:
