Annual Performance Evaluation

  • For this transition year, we will extend the current performance review period through August 31, 2020. Therefore, this year’s performance evaluations will cover the period February 1, 2019 – August 31, 2020, and the forms will be due to Human Resources on October 1, 2020. We strongly encourage supervisors to continue meeting with employees regarding their performance over this extended period.
  • Evaluations are due to HR by October 1, 2020.
  • The supervisor must place the original evaluation form in the employee's departmental personnel file and provide a copy to the employee and a copy to the Office of Human Resources no later than October 1, 2020.

Delivery Method:

Once the supervisor and the employee have discussed the performance evaluation the supervisor must:

1) Obtain all digital signatures (once one person has signed the document, the form will lock and no changes can be made)

a) To obtain a signature, click on the appropriate box to sign
b) Once you have signed, save the document using the “Save As” function; do not change the file name
c) Forward the document to be signed by the next person and repeat steps 1a and 1b
d) Once all parties have signed the document return the document to the primary supervisor

2) Create a PDF of the Performance Evaluation (the PDF document title is the employee’s 10-digit Employee ID)

3) Place the original evaluation form in the employee’s departmental personnel file

4) Provide a copy via email to the employee

5) Place a signed copy of the PDF file in your assigned SharePoint file; if you do not know the SharePoint file please reach out to your     HRBP

Probationary Employee Evaluation

A newly appointed regular classified employee who is on probation as described in UTSA Handbook of Operating Procedures: 3.06 Probationary Period For Classified Employees will require periodic performance evaluations at 30, 90 and 160 day intervals.  The appropriate forms to be used for this purpose are listed below:
