Change of Major Appointment Request Form

Policies & Procedures

Please read the guidelines listed below:

  • This form must be completed by the UTSA undergraduate student who requests a change of major. Graduate student advising can be found here.
  • This form is only for undergraduate students interested in changing their major. To change your minor, concentration, and/or specialization, contact your assigned Academic Advisor.
    • Note: You must be a current UTSA student. If you have not attended your academic advising appointment for new student orientation, do not submit this form.
  • Your major will not automatically be changed by submitting this form. An appointment with an Academic Advisor is required.
  • Online Degree Options: Please note when selecting your major that some majors have an “Online” designation. This means the degree is 100% online. Learn about these online majors.
  • Please allow 5 business days to receive an appointment notice. If you have not received a notification within 5 business days, please contact either:

  • Please complete all sections of this form. Incomplete or inaccurate requests cannot be processed.
