How to Replace a Barcode

Equipment barcodes may be damaged or lost after a period of time. Replacing the barcode ensures equipment has a readable barcode for annual scanning. To request a replacement barcode, follow these steps:

  • Download the Barcode Replacement Request Form
  • Complete the form and obtain appropriate signatures
  • Email form to the Inventory Department
  • Inventory staff will coordinate a date and time to physically verify and affix the replacement asset tag
  • Keep a copy of the form for your records
  • Provide a copy of the completed form to your department’s Inventory Contract Person for departmental records
