Replacing Your One Card
If your One Card was lost, stolen, damaged or you have a name change, you must submit a Card Maintenance Request form using DocuSign* for processing and/or to simultaneously request a replacement card.
Be cognizant that lost or stolen cards must be reported to Citibank immediately to reduce the department’s liability of incurring unauthorized charges. We may ask for additional information when we receive your request.
